It focusses on an androgynous species where identifying as a gender was seen as subversive. Star Trek morality tales are usually as subtle as a sledgehammer, and this is no exception. And it's really not a very good episode.
It's a clumsy metaphor for homosexuality. However it kind of works better as a clumsy, heavy handed metaphor for gender identity, and I'm sure that was barely even a conversation in 1992.
